Incident response writeups are sociological stories, not stories about individuals. They should describe the systems as a whole, where they failed and where they succeeded1.
Lorin Hochstein discussed how he as a write up style for lower grade of incident called an OOPs which is worth considering for any degree of incident writeup2.
