At Netflix, there is a project called OOPs which tracks operational suprises: when the system is behaving in an unexpected way. These OOPSies are written up and tracked in a very similar way to incidents with cavaet that OOPS do not need to be high severity / high impact in the way that incidents are1,2. He presents a template for how he writes up these incidents3.
